I agree that the information provided in this application form may be processed by Enable Therapy Services (ETS) to assist in the decision making process. I further expressly agree that, should it be necessary to validate any of the information provided herein, ETS may release this information for verification purposes.

I authorise ETS to obtain references to support this application once an offer has been made and accepted, and release ETS and referees from any liability caused by giving and receiving information.

If successful in my application, it is agreed that any information provided will be retained by my express consent for disclosure where necessary. I am not currently included (either provisionally or fully) on the list maintained by the Secretary of State for Health of persons considered unsuitable to carry out care work. I give my consent for disclosure of an appropriate criminal record certificate.

I understand that any offer of employment will be subject to the information on this application being complete and correct. Any false information or failure to supply any details required could make an offer of employment invalid or lead to termination of employment.

CONFIDENTIALITY AGREEMENT – FOR USE IN EXPLORATORY DISCUSSIONS PRIOR TO JOINING ETS AS AN ASSOCIATE

ETS, intends to disclose information (the Confidential Information) to the Recipient for the purpose of sharing information about becoming an Associate with ETS and, if successful, when contracted to work with ETS, as an Associate on the ETS Associate Network – herewith referred to as the Purpose.

2. The Recipient undertakes not to use the Confidential Information for any purpose except the above Purpose, without first obtaining the written agreement of ETS and the Recipient also agrees to keep the Confidential Information secure and not to disclose it to any third party.

3. The undertakings in clauses 2 will remain in force indefinitely from the date of this Agreement and the Recipient will never disclose the information received from ETS unless express permission is given in writing.

4. ETS undertakes not to utilise the Recipient’s CV until the point the Recipient joins the Network formally or at the point at which the Recipient provides prior agreement, if appropriate.

5. Without prejudice to any other rights or remedies that each Party may have, each Party acknowledges that any breach or threatened breach of this Agreement may cause serious damage. This Agreement is governed by, and is to be construed in accordance with, English law. The English Courts will have non-exclusive jurisdiction to deal with any dispute which has arisen or may arise out of, or about, this
Agreement.
